The Embassy can issue a letter informing that an individual is taking medication while travelling to Indonesia. The letter is neither for legality purpose nor providing guarantee that you will be exempted from any checks and legal consequences that may arise. It will only provide information to Indonesian authorities that an individual is under medication based on the letter of a doctor and the drug prescriptions.

Please make sure that the medicines are in original packaging and within the reasonable amount considering the length of stay in Indonesia.

Requirements

  1. A completed and signed application form (downloaded here).
  2. Copy of passport.
  3. Itinerary ticket (roundtrip).
  4. Original letter from the doctor (or copy of the letter certified/ authorised by a Justice of the Peace (JP)).
  5. Original prescription (or copy of the prescription certified/ authorised by a Justice of the Peace (JP)).
  6. Receipt from pharmacy.
  7. Fee : AUD 30.00, by Money Order, payable to Indonesian Embassy For application by mail, please read carefully

The letter is neither for legality purpose nor providing guarantee that you will be exempted from any checks and legal consequences that may arise. It will only provide information to Indonesian authorities that an individual is taking animal and it is has been checked by quarantine and Australian Animal Health Laboratory.

  1. A completed and signed application form (download here).
  2. Copy of passport
  3. Itinerary ticket (roundtrip)
  4. A letter from Australian Animal Health Laboratory (or copy of the letter legalized by GP).
  5. A letter from quarantine
  6. Fee : AUD 30.00, by money order, payable to Indonesian Embassy.

The Quarantine Office of the port of entry should be notified of the time and date of the animal\’s arrival so that inspection of the pet can be arranged upon arrival. Owner/importer or appointed agent should be responsible for the pet upon arrival at the port of entry, and should finalize the arrangement by requesting an examination by a veterinarian at the Quarantine Office at the port of entry.

Requirements

  1. A completed  and signed application form.
  2. A letter to the Ambassador of the Republic of Indonesia clarifying the purpose, amount and the recipient of gift/donation, as well as other relevant information.
  3. A gift certificate explaining that the donation/gift would be granted to an individual or an agency/institution in Indonesia.
  4. Invoice and packing list of the goods. At the end of the invoice/packing list, please insert this statement “no commercial value and value for custom purpose only”.
  5. Enclose correspondences between donor and recipient.
  6. Letter of approval from relevant authorities in Indonesia.

Starting on 9 October 2012, The Indonesian National Police  SLO/ Police Attaché office is temporarily not accepting  applications for INP police clearance.(Complete applications received prior 9 October 2012 will be processed normally).
